Mystery box perma perk and another question

I've been asking and searching. The first time I played this map and found the box, I hit it and got the green smoke immediately first try and then had the teddy on top for awhile after that. I've since lost it and haven't been able to re obtain it. Another question I have is, is the holding candy trick true about only losing 1 perk when going down and being revived? I'm very curious about both of these. Thanks guys

I've noticed with the mystery box, if you get a bunch of bad weapons (and pick them up) the teddy bear comes and it gives you good weapons, and vise versa. As for the going down with candy, I don't know
