Have you pressed the button in the courthouse with three other people who have also completed all three side quests for Richtofen, and have also inserted their Nav Cards? Or, I guess, does the button show up at all?

As long as all 12 "main" lights are all one colour and the 3 NavCard lights are lit then you've done everything right, I think

Yeah its a bit of a pain and doesn't make much sense.... The simple fact that ONE person has done it should mean the tranzit characters in-game have done it....

Also another question, if you do the endgame do the effects carry over to the next game? (Not infinite fire sale and stuff like that, but things like Richtofen/Stulingher and the zombies eyes going yellow).

Nope... Although they SHOULD carry over into dlc 4's map....
