To get certain medals, you have to meet a certain criteria for each medal. The list of every medal on the site can be found here:

viewtopic.php?f=115&t=20938

Once you find the medal that you can apply for, just head over to this page and post your request:

viewtopic.php?f=115&t=22055

Make sure that you post an image containing the proof that you are able to obtain that medal. Some of the medals require in-game photos, while other can be achieved through the Call of Duty: Elite site.
